How do I find a provider?
Once you have enrolled into one of the dental plans, you can locate a provider by registering with Delta Dental of Arizona here.
How can I get another dental ID card?
Delta Dental of Arizona will mail your ID card to your home address after you have enrolled. If you lose your card you can get a replacement by contacting:
